Student Health History Information

Each year, a parent or guardian will need to complete a Student Health History Information form. This form helps connects the dots between academics and health for each student we serve at Lomega Public Schools.

Medication Administration Form

Lomega Public Schools requires a medication consent form for any medication to be given at school. This includes both prescription and non-prescription medications. If your child has an asthma, anaphylactic, or diabetic medication, there may be additional information sent home with the student after the completion of the Medication Administration form.

Oklahoma Emergency Guidelines for Schools

These Emergency Guidelines are for use in the Oklahoma Public Schools. The guidelines have been created as a recommended procedures for when advanced medically trained personnel are not available on the school site.

Wednesday is "Class Color Day" for JH and HS. Elementary students is "Dress Your Best" (Picture Day).
