Medications at School

Medications at School

It is the responsibility of the parent/guardian to inform school personnel of any medical conditions and the medication that the student is taking that may have an effect on their child’s educational success.  If it is necessary for medication to be given during the school day, compliance with the following instructions is required.

A Parent/Guardian must bring the medication (Prescription or OTC-Over the Counter) to the school and picked up the medication at the end of the year. Medications left at the end of the school year will be destroyed. This not only protects your child, but the staff who will give the medication.

It is encouraged that parents give their child’s medication at home on a schedule outside of school hours. For example, if the medication states daily – you may give before school or at bedtime, or if the medication states two times daily – you may give the medication before school and at bedtime, or if the medication states three times daily – you may give the medication before school, after school, and at bedtime.

A signed Medication Authorization Form is required on file with the school for all medication. Please see below for link to print and complete to return to the school.

The following is REQUIRED for all Prescription Medications: A Pharmacy labeled bottle with the Name of the Student, Name of the Medication, Dosage Amount to be given, Route of medication to be given, Time to be given, Physician Name, and reason for medication. Your pharmacist will be glad to provide you with an extra bottle and label for this purpose.

Elementary Students – All medications to be given at school must have a signed Medication Administration Form on file and be kept in the school office, regardless of the student’s age.

JH/HS Students – All medications to be given at school must have a signed Medication Administration Form on file and be kept in the school office with the exception of emergency medications (such as inhalers, EpiPens, and insulin), which may be carried by a student AFTER the school receives the completed Medication Authorization Form with signatures from the parent AND physician, stating that it is necessary for the medicine to remain with the student due to facility locations.

Requests from a parent/guardian to change the dosage of any medication beyond that listed on the label will not occur without written confirmation from the prescribing physician/dentist or an updated prescription bottle is provided to the school. In addition, a new Medication Authorization Form must be completed for any change in medication, dosage, time, or duration. Telephone permission will not be accepted.

Only FDA-approved medication will be given at school. Herbal and homeopathic supplements will not be given at school.
